Your First Visit

First of all, we want to thank you for choosing Chalet Dental Care.  We are confident that your experience here with us will meet and exceed any expectations you may have.  You will find a wonderful, caring staff that will go above and beyond to make sure that your needs are met.  We are a family oriented practice and we are confident that you will feel apart of this ever growing family as well.

We will give you a phone call a day or two prior to your appointment to check in and see if you have everything you need and answer any questions you may have.  If any contact information changes prior to this time, please shoot us an email or give us a call so we can update your information.

Your initial appointment will consist of any necessary x-rays and a consultation explaining your diagnosis and treatment options. Occasionally, treatment, such as a dental cleaning, can be performed the same day as the consultation.  However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

• Any current x-rays (if applicable)
  • Bitewing x-rays within the last year
  • Panoramic or Full Mouth Set of X-rays within the last 3-5 years
• A list of your current medical prescriptions
• If you have dental insurance, please bring in your insurance card or information

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Please alert the office if you have a medical condition that may be of concern prior to your first appointment (i.e. artificial heart valves and joints, rheumatic fever).

X-Rays

If your previous dentist has taken x-rays, you may request that they forward them to our office. If there is enough time, please pick them up and bring them to our office on your first appointment.  If additional films are necessary, they can be taken at our facility.

If the x-rays are digital they may be e-mailed to chalet@chaletdental.com

When you first visit the Chalet, we will ask you to complete a registration and medical history form.  We will take the time to understand your dental history and ongoing needs.  If possible, please Open and Print each form below.  Complete it and bring with you to your first appointment.  If you would like to email it, please do so to chalet@chaletdental.com with your name and appointment date as the subject heading.
