Insurance & Financial Policies
We understand that in today’s economy, saving money is an important consideration for all of our patients. We will try to provide alternative treatment options for each situation, but feel an obligation to caution against price-based decisions. Many times short-term dental fixes with lower fees end up costing more over time. Quality dental work is an assurance of long-term VALUE and requires both a financial commitment and a commitment to treatment time.
DENTAL INSURANCE
As a courtesy to all patients we will verify your insurance benefits and enter them into our dental software. Although we are able to maintain computerized histories of payment and benefits of a given company, they do can and do change, therefore we will estimate the patient portion based on the most up-to-date insurance benefits. Our Patient Care Coordinator will utilize this information to develop a detailed TREATMENT PLAN that will outline the service fee, insurance and patient portion.
By accepting most insurance plans it means that we work with literally thousands of companies. It will be your responsibility to keep your insurance current with us and to know your plan coverage, exclusions and limitations.
Furthermore, you should make yourself aware of any non-covered or downgraded benefits including but not limited too:
- missing tooth clauses
- crown/bridge/denture restorations
- bruxism or occlusal guards/splints
- downgraded limitations for white fillings
- porcelain crowns on molar teeth
- frequency limitations on exams, cleanings and x-rays
We accept virtually all insurance policies.
We will gladly submit dental claims to your insurance company for payment. This includes those insurance companies that are either In-Network or Out-of-Network Providers.
Dr. Fredrickson, Dr. Rieck & Dr. Smith are Providers for the following Dental Insurance Companies/Networks:
- Delta Dental Premier
- Health Partners (Tier II)
- CIGNA Dental (NOT DHMO Option)
- Dentaquest (UCARE For Seniors Only)
- Dentemax Network ~ Dentemax is the Administrator for many insurance companies
- Premier Dental Network ~ Premier is the Administrator for many insurance companies
Dr. Kingston is a Provider for the following Dental Insurance Companies/Networks:
- Delta Dental Premier
- Delta Dental Preferred Provider Option (PPO)
- Health Partners
NOTE…..We are NOT PROVIDERS for the following insurance plans: Medical Assistance, MNCare, UCARE MSHO, General Assistance Medical Care, and STATE FUNDED PLANS thru Medica, Blue Plus and Delta.
Information provided to Chalet Dental Care from the patients insurance company or patient is NOT A GUARANTEE of benefits or insurance coverage. We do our very best to provide an accurate estimate, but please keep in mind that your dental insurance is a contract between you and the insurance company. It is ultimately the patient’s responsibility to know their plan benefits.
PAYMENT POLICIES
We provide services to patients of many means. This includes patients with private insurance, employer-based insurance, public coverage and self pay. Whatever your circumstances, we are here to provide confidential review of your options.
At Chalet Dental Care we require payment in full for your portion at the time of service, unless prior payment arrangements have been made in advance. If you have Insurance we will require the estimated patient portion due, which is based on the treatment plan discussed above.

- We accept all major credit cards ~ VISA, MASTERCARD, DISCOVER, AMERICAN EXPRESS
- We accept CARE CREDIT which offers little to No-Interest Payment Options
- We offer a 7% discount to all Seniors 60 years and older (entire balance must be paid on day of service)
- We offer a 5% discount to any non-insured patient paying by Cash or Check (entire balance must be paid on day of service)
- We offer a 4% discount to any non-insured patient paying by Credit or Debit Card (entire balance must be paid on day of service)
